TieCare Announces Launch of Mental Health Platform ‘TieCare Connect’: Innovation in Mental Health Access for International Educators, But Is It Enough?

TieCare International has just announced the launch of TieCare Connect, a new mental health platform they’ve developed with 1.800MD, set to roll out for their international school clients renewing after June 1, 2025. According to TieCare, this platform aims to make mental health care more accessible, more flexible, and more affordable for international educators by offering no out-of-pocket costs – no deductibles, co-pays, or co-insurance – when using TieCare Connect.

That’s great to see, however, the shift also comes with a notable restriction: direct billing will be discontinued for mental health providers outside the TieCare Connect platform. This means members seeking care outside the platform must pay out-of-pocket and submit a claim for reimbursement.

At One World Cover, we fully support innovative strategies that aim to ensure easy access to mental health care – especially for international educators, many of whom live far from home and under considerable professional and personal pressure. But we do have some concerns about the rigidity of the new approach.

Mental Health Is a Non-Negotiable Benefit

During the 2024 international school leadership conference circuit, TieCare President Mark Tomaszewski shared that mental health claims can represent up to 25% of total claims at some schools. That number is staggering – and clearly demonstrates the problem TieCare is trying to fix because while adequate access to mental health care is critical, mental health claims are a major cost driver in international school faculty health plans.

We applaud TieCare for not only recognising the problem but actively investing in a solution. TieCare Connect represents an ambitious effort to address three challenges at once:

  • Access: offering care from anywhere in the world via virtual platforms
  • Affordability: removing financial barriers to treatment
  • Consistency: standardising care through a dedicated network.

But at the same time, cost pressure should not lead to eliminating access. At One World Cover, we define mental health care as a non-negotiable benefit. It’s not a luxury, and it should not be on the chopping block during cost reviews.

Removing access to certain types of care – especially by eliminating direct billing – could limit support for those most in need.

Why a Hybrid Model Might Work Better

We’ve helped clients design cost-effective mental health structures that don’t require restricting access. In one example, we supported a school in introducing a US$125 cap per therapy session. That change encouraged more cost-conscious decisions – but faculty could still access higher-priced therapists if they were willing to pay the difference themselves.

That’s a better balance: where possible, steer members toward lower-cost options, but don’t block them from getting the care they trust. Giving staff options – and the freedom to choose based on their needs and budget – is key.

In fact, TieCare has offered some flexibility to their clients for their 2025–26 renewal. In renewal discussions, they presented two options with different pricing:

  1. TieCare Connect only (with lower premiums), with no mental health cover at out-of-network providers, or
  2. A hybrid model (more expensive), which maintains limited cover (for example, capped number of sessions or capped reimbursement amounts) outside TieCare Connect.

We strongly recommend the hybrid approach to our clients. It ensures people are incentivized to use TieCare Connect – with no out-of-pocket costs – while still giving them a fallback option if they need care from someone outside the network.

Its also important to note that some mental health needs cannot be met through telehealth alone – especially when it comes to psychiatric care.

To take one example: children diagnosed with ADHD or other neurodivergent conditions often require medication that must be prescribed by a psychiatrist, not a therapist. If in-person psychiatric visits are no longer supported under the plan with direct billing, these children – and their families – could be left without essential care.

Mental Health Support Should Not Come at the Cost of Choice

In our work with international schools, we see the need for both innovation and flexibility. TieCare Connect is a commendable effort to address the major challenge of spiralling health insurance costs. But in trying to control access, there’s a risk of going too far.

We recommend international school HR leaders using TieCare – or any insurance provider serving the international school market for that matter – to push for a hybrid structure that combines in-person/face-to-face mental health care with virtual/telehealth access.

When reviewing your next renewal, ask yourself:

  • Does our mental health plan allow access to both therapists/psychologists and psychiatrists?
  • Is there a cap or limit that helps manage costs without blocking care?
  • Are we restricting access too severely, or incentivizing smart choices?
