Should Flu and COVID Vaccines Be Covered by International Health Insurance Plans?
As COVID-19 becomes a more regular part of the annual respiratory virus season – alongside flu, RSV, and other airborne illnesses – a common question is emerging:
Should we treat COVID and flu the same when it comes to vaccine coverage?
For expat employers committed to employee wellbeing and long-term cost control, the answer is clear: yes – and your health insurance plan should reflect that.
COVID and Flu: Now Part of the Same Landscape
In many countries, COVID-19 is no longer seen as an isolated crisis. Instead, it’s now managed like the flu: recurring, seasonal, and preventable – at least partially – through regular vaccination.
Both viruses:
- Spread more easily in enclosed spaces (especially relevant for schools and offices)
- Show similar symptoms (fever, cough, fatigue, body aches)
- Pose heightened risks to older adults, pregnant individuals, and those with chronic conditions
- Lead to avoidable hospitalizations and medical claims if not managed early
For employers with mobile, international teams, the potential exposure to flu and COVID while traveling or relocating is even higher. Vaccination isn’t just good health practice – it’s risk mitigation.
Vaccines Are a Smart Investment — Not Just a Benefit
Vaccines are low-cost, high-impact tools that help employers reduce absenteeism, boost productivity, and avoid expensive claims. The average cost of a flu vaccine is under US$50. The cost of a hospitalization? Thousands – sometimes tens of thousands – of dollars.
From a cost containment perspective, offering and encouraging access to both flu and COVID vaccines should be a no-brainer. In fact, many hospitalizations from both illnesses are preventable with annual boosters.
Some of our international insurer partners recently questioned whether COVID vaccines should still be reimbursed, citing a shift in public sentiment. That’s a short-sighted approach – and one that risks increasing both health risks and claims.
At One World Cover, we believe coverage for both flu and COVID vaccines is critical and non-negotiable in a robust international health insurance strategy.
Who Should Be Vaccinated?
According to global health authorities such as the CDC, WHO, and NHS:
Flu Vaccine:
- Recommended annually for everyone aged 6 months and older
- Especially important for:
- Adults over 50
- Pregnant women
- Children under 5
- Individuals with chronic conditions (such as asthma, diabetes, heart disease)
- Health workers
- Globally mobile employees, expats and frequent international travelers, due to increased exposure to varied flu strains and seasonal transmission risks
COVID Vaccine:
- Annual boosters recommended for:
- Adults over 60
- People with weakened immune systems
- People with chronic illnesses
- Healthcare workers and those in close-contact professions
- Globally mobile employees and expats, especially those who travel frequently or work in high-contact international environments
Can the vaccines be given together? Yes. In most cases, COVID and flu vaccines can be administered at the same time, often in separate arms. Many public health systems encourage co-administration during peak respiratory season.
When Should Vaccines Be Administered?
Timing is key. For maximum protection, vaccines should be administered just before the start of the respiratory illness season, typically between September and November in the Northern Hemisphere.
This ensures that employees are protected during winter months, when flu and COVID cases typically spike.
Why This Matters for Expat Employers
If you’re employing globally mobile staff, their exposure to viruses is higher than average. International air travel, border crossings, and short-term housing all increase the risk of catching and spreading respiratory illnesses.
Health insurance coverage that excludes or limits access to seasonal vaccines is not just a wellness oversight – it’s a risk to your people and your bottom line.
We recommend employers ask:
- Does our plan cover both flu and COVID vaccines annually?
- Can staff get vaccinated at any provider, or only at limited clinics?
- Do we communicate vaccine access and encourage uptake?
If the answer to any of these questions is “no,” it’s time to talk to your broker or insurer – or find one that can better support both cost containment and employee wellbeing.
